MOLST ManualThe MOLST Program: A Community Approach to Improving Care at the End-of-Life on-line training curriculum is updated to align with Family Health Care Decisions Act (FHCDA), effective June, 1, 2010 and the revised DOH-5003 MOLST form.  Decision-making standards, procedures and statutory witness requirements for decisions to withhold or withdraw life-sustaining treatment, including DNR, vary depending on who makes the decision and where the decision is made.

Patricia Bomba, M.D., F.A.C.P., a national expert, has presented regional train-the-trainer conferences across New York to assist in statewide implementation of the MOLST Program since 2005.  In collaboration with the MOLST Statewide Implementation Team the National Healthcare Decisions Day NYS Coalition, she is developing the MOLST & FHCDA series of recorded webinars and slides, along with revised web pages and new resources to replace presentation that are obsolete, as a result of the FHCDA and the revised DOH-5003 MOLST form.

Accordingly, the New York State Department of Health (NYSDOH) has developed different legal requirement checklists for different types of decision-makers and settings.  There are five different checklists for adult patients, one for minor patients and one for patients with developmental disabilities who lack medical decision-making capacity.  If a patient has a developmental disability and does not have the ability to decide, the doctor must follow special procedures and attach the appropriate legal requirements checklists.
